Have an MRI?
Bring it with you.
Your images, written report, and experience of your symptoms belong in the same conversation. This guide helps you organize what you already have for an NSI evaluation.
Bring the images
Ask your imaging center how to obtain a copy of your MRI or X-ray images. If access is through a disc or an online portal, call NSI ahead of your visit to confirm how to share them.
Bring the written report
Include the radiologist’s written report and the date of the study. If you have older relevant studies, bring their reports too so you can discuss what has changed.
Bring your story
Note when symptoms started, where you feel them, and how they affect walking, sleep, work, or other daily activities. Include what you have already tried and how it helped.
No recent MRI? You can still get started.
You can request an evaluation without arranging a new scan first. Ask the team what records to bring and discuss any imaging needs with your provider.

What do my examination and imaging suggest? What remains uncertain? Would any additional information change the recommendation?

Why are you considering this approach for me? What are the alternatives, potential risks, and limits of the available evidence?

What will the visits and follow-up involve? What will I pay? How will we measure progress, and when would we reconsider the plan?
Write down what matters most.
A useful goal can be personal and specific: walking with family, sleeping more comfortably, returning to an activity, or understanding the next step. Bring that goal into the conversation.
