Our team will contact you to discuss your request and appointment availability. Sending the form is a request; your appointment is confirmed when our team arranges it with you.
What should I bring?
Bring available imaging and written radiology reports, a list of treatments you have tried, and your questions. If you do not have imaging, contact our team about getting started.
Can I explore my options before choosing treatment?
Yes. Your evaluation is the opportunity to discuss your concerns, goals, recommendations, and questions about benefits, risks, alternatives, and costs before deciding on next steps.
